Applying for approval of Consent and Moderation Requirements (CMRs)
To apply for approval of a CMR submit an application online using the “Other” category and write in the type of application (e.g. CMR).
The CMR should be submitted using the CMR template (DOC, 45KB).
An application may be for new or reviewed CMRs.
- If the CMR has been reviewed, complete a change report (DOC, 36KB), outlining the change and submit it with the application. It will be published with the reviewed CMR.
